A site to let coaches measure range-of-motion in their athletes, and enter results. Then, later,
coaches can enter any injuries that occur later in the season. Ultimately, this data can then be
mined for correlations, and used to help predict and prevent injuries.
Details: There will be four different movement-quizzes, some fairly involved. There are some other
possible items, but let’s start with these first and see how the project progresses.

==== First Item is Functional Movement Screen (FMS).
form
(1) 21 points possible (Total or Composite score)
(2) Some of them have a right and left side, the lowest score is transcribed and becomes part of the Composite score.
(3) Since some have right and left scoring, want to look for asymmetries between sides (as applicable).
==== Second item is Y-Balance for upper quarter (UQ-YBT) and lower quarter (LQ-YBT).
form
For both UQ-YBT and LQ-YBT there is the following (so two separate tests requiring two separate data entry points, so they are independent data collection sections):
(1) A composite score for right and left sides
(2) There is an Asymmetry component: Lower Quarter = Score (each direction): Should not be > than 4
cm Right & Left reach difference in anterior direction = Asymmetry. Should not be > 6 cm reach
distance in postero-medial & postero-lateral directions = Asymmetry.
(3) There is an Asymmetry component: Upper Quarter = Score (each direction): Should not be > than 4
cm Right & Left reach difference in medial, infero-lateral, and supero-lateral directions.
==== Third items is Balance Error Scoring System (BESS). Again, two separate options that need to be loaded and able to be used independent of each other.
form
form
(1) A Modified BESS
(2) And the standard BESS (which includes the foam pad to stand on)
I have attached both forms to this email. There are multiple entry points on these, so I will let the handouts do the explaining.
==== The fourth item is the King-Devick score, which is probably the easiest.
It does not require a handout or attached form. It only has a final score and yes or no if they wear eyeglasses; see below in RED:
King-Devick (K-D) Score: ____________________________
Eyeglasses: Yes / No
